PURPOSE

Provide guidance to DBS staff regarding information that can be released to a client from their file.

POLICY

Within 10 business days of a client signing a DBS 017 Consent to Release Information form the Division will release all information either purchased by way of client services authorization or directly generated by DBS staff (i.e. plans, case notes). Such records may be subject to confidentiality by provision of law and may only be released under conditions established by the providing entity.  Without the consent of other agencies, only the name of the providing entity, its contact information and a simple description of the records obtained may be released. The Division will not charge for these services and will provide the material in the client’s chosen format.
